You can better assist us in helping you if you bring the following information with you to your visit:
Referral slip along with name and number of your general/referring dentist
Any current x-rays
A list of medications you are currently taking
Current insurance information including:
    - Subscriber ID #
    - Group policy #
    - Social security # (if your insurance requires)
    - Subscriber's date of birth
    - Insurance card (if applicable)

Insurance

At Best Endodontics of Mt. Prospect, we accept most insurance plans and select PPO networks. Due to the extensive and complicated nature of insurance coverage, we recommend that you call our office to verify that we accept your insurance provider.

We make every effort to provide you with the finest care and the most convenient financial options. To accomplish this goal, we work hand in hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, contact our office administrator at 847-398-0404. Also, please remember to bring your insurance card to your appointment.

Financial Policy

FEES
The fee for your treatment will be based on the type of treatment necessary and the extent of treatment.

PAYMENT
We accept cash, check, or credit card (Visa , MasterCard, and Discover). For patients with insurance coverage an initial payment, co-payment, or full fee may be required at the time of service depending on the coverage of your individual insurance policy. For patients without insurance, it is our policy that your treatment is paid for at the time of service. In certain circumstances, arrangements can be made on your behalf establishing a payment plan.

SUBMITTED INSURANCE CLAIMS
We will send all claims to insurance companies, in network, or out of network as a convenience to our patients. Insurance companies offer many different dental plans. Most dental insurance policies are limited and often pay only for a portion of the procedure. Very few companies pay for the entire claim. It is best for you to check and understand your individual insurance benefits. Please refer to your insurance booklet for any questions regarding your benefits.

Upon receipt of the initial payments, co-payments, and/or insurance payments, any balance due will be billed to you. If you have an overpayment we will refund you the difference. Please understand we cannot guarantee coverage and/or be held accountable for any denial of insurance coverage.
